Wanting to become a new patient?

• Need Referral from who has been treating your pain this past year
• Need Insurance Card
• Need Office notes that discuss your pain and history
• Any Diagnostic Imaging
• Request your doctor to send this information to our fax number of 502-290-7252 or email us the referral at scheduling@vitalitypain.com
• Once we get this information will give you a call to get scheduled

 

Already an established patient with us?

• If you have not been in here in over a year, will need a new referral sent from your doctor
• If you have been here less than a year please call us at 502-290-7246 to further assist
